Under the Well-Led key question, inspectors assess the quality of governance -- including governance of the physical environment. Strong property governance evidence includes: a five-year forward maintenance register reviewed at board level, monthly facilities management reporting, a documented system for logging and tracking maintenance defects to resolution, evidence that the registered manager understands and can speak to the property compliance position, and board minutes showing regular oversight of property risk.
Inspectors look for evidence that the provider learns from what goes wrong. For property, this includes: incident reports for environmental incidents (falls due to floor defects, Legionella detections, fire door failures), root cause analysis, and evidence that system or process changes have been implemented following incidents.
Well-led also requires honest self-assessment. A provider that consistently rates itself as Outstanding in its own quality monitoring while CQC finds multiple concerns on inspection demonstrates a governance failure. Annual self-assessment of the property and compliance position, calibrated against independent inspection findings, is evidence of sound governance.
