CQC assesses care homes against five key questions: Is the service Safe? Effective? Caring? Responsive? Well-led? The safe and effective domains are particularly relevant to the physical environment. Evidence of a well-maintained, safe building with documented systems and processes contributes to ratings across multiple domains.
Since 2023, CQC has been rolling out its Single Assessment Framework (SAF) -- a significant change to how inspections are carried out and how ratings are determined. Under the SAF, evidence is collected continuously from multiple sources (provider records, observations, feedback from people, staff interviews) rather than primarily during a scheduled inspection visit. This means the quality of your record-keeping and systems matters every day, not just on inspection day.
Under the SAF, CQC assesses against 'quality statements' -- specific commitments about how care should be provided. Quality statements relevant to the physical environment include: 'We provide safe environments, free from hazards' (Safe), 'We ensure equipment and premises are maintained to support good care' (Effective), and 'We create environments that support wellbeing and feel homely' (Caring).
CQC expects to see evidence, not assertions. Saying 'we maintain our building well' is insufficient -- inspectors will look for maintenance logs, contractor certificates, completed checklists, risk assessments with review dates, and action plans showing how identified issues have been resolved. The quality of evidence is as important as the quality of the physical environment itself.
