NHS HTM 03-01 is the benchmark standard for ventilation in health and social care settings. Most care home maintenance programmes fall short. Here is what full compliance actually requires.
NHS Health Technical Memorandum HTM 03-01 (Specialised Ventilation for Healthcare Premises) sets design and maintenance standards for ventilation in health and social care settings. Whilst it was developed for NHS estate, it is the benchmark document referenced by CQC, UKHSA IPC inspectors, and infection prevention teams when assessing care home ventilation adequacy. A care home that cannot demonstrate its ventilation maintenance meets HTM 03-01 principles is at a disadvantage in any IPC-focused inspection.
Most care homes contain several different ventilation systems, each with different maintenance requirements: mechanical extract ventilation (MEV) in bathrooms, toilets and sluice rooms; kitchen extract canopy and ductwork systems; whole-house mechanical ventilation with heat recovery (MVHR) in newer builds; and, in some buildings, specialist isolation room ventilation. Natural ventilation dominates in older stock. Each system has a maintenance schedule -- and the mistake most operators make is treating ventilation maintenance as a single line in a contract, covering all systems with one visit and one fee.
Kitchen extract systems accumulate grease in the canopy, duct filters and ductwork. Grease is highly flammable. A poorly maintained kitchen extract system is a significant fire risk -- the ductwork becomes the fuel. The BESA TR/19 Guide to Good Practice for Internal Cleanliness of Ventilation Systems sets the standard for duct cleanliness. Kitchen extract systems should be deep cleaned and inspected at least annually (quarterly if the kitchen operates at high intensity). The contractor must produce a TR/19 report after each clean confirming the cleanliness standard achieved -- not just a service visit note.
Where ventilation ductwork penetrates fire-rated walls and floors, fire dampers maintain compartmentation. Under BS 9999, they must be drop-tested annually. In practice, this is almost never included in standard ventilation maintenance contracts. The result is that care homes may have dozens of fire dampers that have never been tested -- a direct failure of the building's compartmentation strategy. Fire damper testing requires a specialist and produces a per-damper test record. It is a separate contract deliverable, not an add-on to the general ventilation service.
Extract fans in bathrooms and toilets should be cleaned and serviced annually. Critically, airflow rates should be measured against the design specification. A fan that is running but not achieving the designed extract rate is not performing its infection control or condensation prevention function. Replace fans that are undersized for their room or at end of life -- this is a maintenance action, not an optional improvement. Airflow measurements should be recorded in the service report.
Mechanical ventilation with heat recovery systems require 6-monthly filter replacement, annual duct inspection and periodic heat exchanger cleaning. Blocked filters reduce airflow and heat transfer efficiency. Dirty heat exchangers can harbour bacteria and mould and distribute them throughout the building. The manufacturer's maintenance schedule must be followed and retained as evidence. Check whether your MVHR manufacturer has published a specific maintenance protocol and incorporate it into your planned maintenance programme.
A complete ventilation maintenance evidence file should contain: TR/19 reports for kitchen extract cleans, fire damper test records with each damper identified by location and result, airflow measurements from extract fan services, MVHR service records including filter change dates, and any remedial works arising. This file is the evidence for CQC, FRS and UKHSA IPC inspections. If any part of it is absent, you cannot demonstrate compliance.
